This was weird -- I upgraded from 4.5.2 (actually it was CivicSpace 0.8.0.3) to 4.6 and all of my taxonomy terms were triplicated. What I mean is that I had a vocab/category called Content with terms "News", "Opinion", "Prose" and under Prose terms "Article", "Essay" and so on. After the upgrade I had three News terms in my list, three Opinion, three Prose and each Prose had three Article, three Essay, every term became three terms. If a term had synonyms, it got triple synonyms.

Even weirder, the three terms all have the same number; editing one entry edited all three, and deleting one deleted all of them. Fortunately fixing it was simple: just edit a term (add a space in the description or something), save, and voila back to one term.

But it was weird, I tell you! And alarming.

My Drupal has exactly the same problem described in this post. With this patch, I also saw three options but all blank.
