Patch to add note to outgoing message

This patch adds a "note" field so that the user can add a little bit of text to the email message getting sent out.

The patch also adds the page title above the link in the email and ensures that the sent url is the 'path' version if that exists. It also returns the user to the (sent) page with a message at the top saying that the message was sent.

Everything seems to be working fine in Drupal 4.6...
