I applied the patch, so far no problems in 5.x.
(http://drupal.org/files/issues/comment-db_rewrite_sql-128165-12.patch) specifically, there are multiple patches listed and this one worked for me on 5.6.

That said, the patch doesn't allow you to hide anonymous comments. I'm not familiar enough with the code base to know if anonymous comments have a unique id and so on and why I'm putting this in feature. But the ability to hide anonymous comments by trusted users would help grow the site by allowing anonymous comments at the same time clean it up considerably.

Even though this is listed for 5.x I'll say for 6.0.

The Karma module has since changed maintainers and headed in a new direction. So I am closing this issue.
