Is the second time that I see this error happening at drupal.org website.
I report this here because I don't saw this error in any other drupal powered website.

This is e´very serious thing. Got this on my drupal site as well for now. Forums are full of this error.

It seems it was something reported several time in 2005 / early 2006:
http://drupal.org/search/node/Duplicate+entry+'cache_flush'

whatever it was, it most certainly has been fixed since.
