When Private Messaging is installed and a user visits another user's profile to shake hands, the prompt tells the user that he has three choices: use Handshakes, send a Private Message, or cancel. The message remains unchanged even when shaking hands with a user that has disabled Private Messaging, though the button to send a message is not available.
The first attached image shows the Private Message settings that disable it, and the second shows the Handshakes prompt.

rconstantine commentedAh, OK. That should be easy enough. For a second I thought you meant I was still allowing that option to take place, not just showing the message.
