Developing a simple online quoting system module

I need to know if anyone has a working solution for a simple online quoting system. The system requires the user to input about 10 quantities and these quatities will be used to determine the total amount to be payed. A bonus would be if the quote could then be saved to the database along with a reference number.

Also if you dont have a working solution, a point in the right direction will be greatly aprreciated.
