I think the annotation form should be in the annotation tab (below all the annotations), just like if it were a comments page with the comment form in the end. This would centralize all annotation activity in the same place and it would be a way to avoid theming problems (how and where in the node page will we put the form or form link?)

It depends on the use case one wants ...

This module annotate is for making quick private annotations on allowed pages.

Making annotations as part of a workflow is provided by annotate_bl ... it should work within a organic group or any other access control modules. Annotate Block Level is placed on a tab for this very same reason.

So this is a won't fix for annotate (private) but certainly not for annotate_bl.
