I've made some progress on this but the code is broken at the moment and I need to go to bed. I'll try to upload a working but still incomplete patch tomorrow.

I'm working on threading at the moment. I can't think of a way to make this easy for people, but I've made a depth field available which I believe will allow one to theme this in a nested fashion. Perhaps a style plugin needs to be made for this. hmmm
