In order to work, SWFAddress needs three extra JavaScript files installed in its module directory. Download links for all three are available off of the SWFAddress module page.
Here is a screenshot of all three libraries installed in the proper place. (Note that your site's status report should also throw up warnings if these files are missing.)

Missing sub-directory and wrong name
I was having trouble getting swaddress to recognize my install and I think I found the issue.
As of SWFAddress 5.x-1.1 the actual install path for the swaddress javascript files should be sites/all/modules/swaddress/swfaddress-2.1/js/*.js
