I am sure this is more of a permissions issue than a "site notes" specific issue, so please forgive my posting. However, I have searched the forms to no avail. any ideas on settings? I currently have the web-root set to 777 this did not fix the error. So, I am not sure where to go next.
I get this error when attempting to create a new site note.

nancydruHmm, Sitenotes doesn't do a fopen by itself. I'm not sure where it would be called. Does the message indicate which directory it is trying to open?
The only thing I can suggest beyond that is to disable other modules one at a time until the problem goes away. The last one you disabled would be the culprit and would give you a better idea of which directory to change.
I do know that some hosts set the "files" directory (and sub-directories) to 644, but that won't work. I change the permissions to 777.
If your host is one of those who default you to "safe" mode, check with them on how to turn that off.
