Hi,

When testing this module, i noticed that the form that pops up when trying to set permissions for a field, doesn't work in Internet Explorer.
I am able to see the 3 radiobuttons, but they cannot be selected.

I tested the generated HTML separately in an empty HTML page, and the radio buttons weren't selectable.

Attached is a patch that solves this, but providing a 'name' attribute for the radio buttons.
