Could you provide an example of the info that should go in the server address field?

Do you mean in the help text of the form, or here in this issue?
It would be the name of the server you connect to to get your IMAP e-mail. For example, 'mail.example.com' or 'example.com' or '12.34.56.78'. It depends on your service provider.
