I got following error when I ran the site documentation. I am not technical enough to comment on the exact nature of the problem. But happy to provide any details on my install...

Please turn on "Include Table Summary." Then go down to the "filter_formats" table and click on "show contents" and attach a screen shot of that for me. I think you have an input format with no roles.
