Can it be programmed so that if a user subscribes to a book's parent node, they would get notified if comments were posted on any of the child-pages as well as the parent node?

It can definitely be done but I am not too sure about how popular the need is going to be to be part of this module. You are basically requesting to track the tree structure of a book and then notify authors about comments to nodes and their children. Comment subscribe currently only tracks the tree structure of comments of a node. We could of course create a cusom module that does that for you.
