Is there a way to include this application on the registration form to create a new account?

Yes there is a way to have this option. The module form fields and actions needs to be added in user registration form via hook_user. If you want this feature really soon and you are willing to pay a bit send me a private message.

yes, and if it could set default roles on registration that would be nice too. Such that a person could apply for role 'bar' on registration, but be automatically assigned to roles 'authenticated user' as well as 'foo' upon email registration, and then have the administrator select final 'bar' membership at a later date.
