Error on content type save

First of all, this is a great idea. The combination of depenent fields and this is bound to be a nicer way to let people choose the proper subform to fillout based on answers.

Ok here is the scenerio:

Relativity setup: 1 parent (dependent), 1 child form(sub_dependent). Content type settings for content type "dependent": I have tried both a single on/off check box and multiple check boxes/radios. When I save the settings on the content type edit page I get this error.

user warning: Column count doesn't match value count at row 1 query: INSERT INTO field_dependencies VALUES...

When I return to the content type settings page "Field instances" setting is set to 'no field'. The create content form shows the subform but there is no control over it.
