Location and Gmap modules not working

I am trying to use the location and gmap module to setup user locations on a google map..

I enable the "User Location " block and then follow the link

Then I get a google map and click on "edit your location" link

But next page I get is blank with 2 buttons: "Submit" & "Delete" and no fields or map onto where I can select my location..

Can anyone point me out the problem?
