I've had major issues trying to append actions to workflow transitions in admin/build/workflow/actions/1.
This involved mostly with the actions for emailing user roles, but I could reproduce the issue with "unsticky" action and with some OG actions.

The problem basically is that actions that you try to add to a transition will not be added and will go to the very first transition in your table instead, here is the log that I made while playing with this:

-when adding an action to a transition fails, the action will be applied to the first transition in the table
-changing weights of wireframing state weights to different values, hopefully solving issues with workflow actions
-this only changed the problem, now problem is only with "notify mage devs" action
-changing mage state weight from -2 to -4 solved problem
-problems arise again when adding the actions to the next batch of transitions
-"mage theme ready for wireframing" email action succesfully placed when trying it 2 times, leaving the first action at the very first transition
-repeating the add-action operation doesn't always work
-editing a workflow state and saving it (without changes) helped
--didn't help again
-after removing all the actions that were erroneously placed at the first transition, an action finally stuck to the right transition for the first time
--this facilitated the addition of 1 other problematic action and then there were more failures
-this last action did stick after deleting the erroneous action from the very first transition (where it was added in error)

p.s. I did finally manage to add all the actions in the right place, I think the "removing action from the first transition" operation never failed me, though I only used it 3 or 4 times.

Hi,

With the release of Drupal 7, Drupal 5 is no longer receiving security updates, reviews, or development from many contributed modules. Since 5 is now considered a depreciated version, you really should seriously look into upgrading to Drupal 6 or 7. The newer versions of Drupal work better, have more support, and will be safer (literally! security patches!) for your website. We are currently working on a new release for Workflow to Drupal 7. In light of that, further support for Drupal 5 issues is infeasible at the moment. Please consider upgrading to Drupal 6 or 7 in the near future - you'll be glad you did.
