Code cleaning and optimizations

There's some cleanups and optimizations

- menu title should be without t()
- parameter for edit_fotonotes is already loaded $node
- use theme('image', ...) against bad
- fotonotes_bbcode - check access for image-node, now works with node_access modules and checks published status
- code style coder module suggestions

There are some bugs in drupal.fndb.inc caused main library
- update is not parse xml so title and content are not works
