I've been chastised to RTFM: http://drupal.org/node/59 very clearly states:

If you are applying for a CVS account because another module maintainer has agreed for you to become a co-maintainer of their module (or to take over the module completely), please ensure this agreement is publicly available by creating a issue in the project's issue queue that clearly shows the current maintainer agreeing to that fact.

Jeremy, if I am to be permitted to help with the maintenance of this project, please indicate so in this issue, and if you will, please state specifically what role you would like me to play and whether you think it's appropriate for me to have a CVS account.

Also, if I have missed further instructions, please direct me to them. I believe I have exercised due diligence in discovering the hoops on has to jump through, but obviously I missed something the first and second times. It seems more than likely that I missed something the third time as well.

Please allow Steve to co-maintain the bookreview module. I have essentially no time left to maintain the module, though I will try to support Steve as he needs it. He will require CVS access so that he can commit changes into CVS.
