It seems to me that it just notifies twitter when I publish something directly from Node/add/blog, it still hasnt notified twitter when I have published some stuff that has been waiting around for me to change from unpublished to published.

this was a major problem for me so I just use http://twitterfeed.com/ now

I just noticed this as well. It would be nice to post on every transition from unpublished to published. Hopefully somebody will get a chance to look at it.

Other than that, I'm really enjoying this module.

Drupal 5 is not maintenance any more, If this issue occurs in any version for Drupal 6 or Drupal 7 reopen this issue.
