/outline does not link properly to nodes in volumes

Description
On the outlines page /outline, the volume_id is used on the item list of volumes instead of the node ids, resulting in a 404 or wrong node when clicked. This patch collects all the top level nodes for each volume and creates titled volume lists for each. Thanks for the module, just what I needed. Enjoy!
