If I delete the Registration Form from list of 'Challenge types per form', how do I get it back?

I can't see any option to turn it back when I configure the registration form.

In fact, I can't see any way to turn on or off challenges on any configuration screen.

Yes, I have selected 'Add CAPTCHA adminstration links to forms'

On which screens exactly do these options appear?

the CAPTCHA administration link are not added to forms on admin pages (more info: http://drupal.org/node/229710)
they are also not visible on forms that are not reachable by admins, such as user registration form (patch for this: http://drupal.org/node/214557)
all other forms (like for example the add comment form) should get the CAPTCHA administation links

A simple workaround for you is to visit yoursite/?q=admin/user/captcha/user_register/enable as admin, this will bring back the user_register form_id
