Despite setting up the PHP connector & the cookie domain per the install instructions, the file manager reports that is not enabled.

It looks like CheckAuthentication() in filemanager.config.php is not loading the session properly, and I'm fairly certain this is because I have installed via profile, hence the module lives in /profiles/myprofile/modules/

FYI, I'm using FCKeditor v2.6 code.

Could you attach your myprofile.profile or write more details?
I have installed FCKeditor 5.x-2.2 under root/profiles/myprofile/modules on D5 and everything work fine.
