I have a suggestion to get this started, but I'm not sure where to put a new book page.

See http://drupal.org/node/134331 -- this appears to be a common problem (just had it myself; see my comment to that post for what worked for me) so a short page in the installation handbook to cover the solutions on different platforms would be useful.
