Unable to create a delicious block

I've installed this module according to instructions (at administer->settings->delicious I've unchecked both smart tagging and crosslinks).

But at step 5 I'm unable to "add del.icio.us block". I can open that tab, and fill in the form - but no block is created. After I've created it the list still says "No del.icio.us blocks exist.". No error messages.

Also, at the next step - enable a delicious username & password at my account->delicious - I don't find anywhere under "My account" to do that.
