Cannot add Field

Issue Summary

Hello
when setting up a new list in members, the 'add field' command does not result in a field added to the list. It just redisplays the same screem from the top.

I have used members before successfully on other sites, no clue why it does not work here. are there any specific requirements (modules, access etc.) for it?
