duplicate audio nodes

whether uploading individually or via audio import module, some audio nodes are duplicating themselves somehow. That is, they show up twice when viewing sites content list (administer > content management > content), or by constructing a View which lists audio nodes.

However, if you try to delete a duplicate, it deletes both duplicates indicating there is still really only one db record, but two views of it.
