I want to create a "form" which contains roughly five field including telephone number, description, postcode.

I want a user to be able to press a button called "Create Item"

This will bring up a form with these fields. When this has been created it will bring up a page saying "Your Item has been submitted".

I'm going to be using gmap and gproximity modules. So I want each 'item' to be node?

How do I style the form(created content) lookout? When the cotent is created wont it load up the content rather than "your Item has been submitted"?

What's the best strategy to deal with this? Is the create node not the right approach? Can a table be created in the database with location fields and then that be used by gproximity?

Any ideas would be great.
