Captcha worked and then the next time I logged on it disappeared. In the admin>modules, everything seemed to be in place for Captcha, but it doesn't appear on any form.

any ideas?

The CAPTCHA only appears for users without the "skip CAPTCHA" permission.
So after you log in as user 1 for example, you won't see any CAPTCHAs anymore
