Sorry if I am being really thick but could you explain in simple terms how to add new registration fields - I just don't know where to start.
Any help would be appreciated.

The signup module has nothing to do with the user registration form (when users create new accounts on your site).

If you're asking "how do I add other fields to the signup form?", the answer sucks:

a) Read the INSTALL.txt file and pay attention to step #2.
b) Read the comment in the signup.theme file above the theme_signup_user_form() function.
c) Copy the contents of that function into a new phptemplate_signup_user_form() function inside the template.php file for your site's theme and edit to taste.
