The [subdomain] token DOES rewrite the path in pathauto, but instead of sub.domain.com/title I get domain.com/sub/title. How do I change this? I've applied the patches directed in the documentation. Anything else I should check?

Yes, you need to edit

pathauto punctuation settings and set "tilde" to "No Action"
