I've had Taxonomy Context running for a few months, I have 4 vocabularies (vid: 2,3,4, and 5), each of them had a Block with the title "Context for Vocab-Name"

Only one of the 4 blocks was active and displaying for Admin users. Today, for no apparent reason, this one (one of the 4 blocks) block just disappeared, it is gone.

This vocabulary has about 500 terms, and growing.

Editing a block takes u to a URL like this mysite.com/admin/build/block/configure/taxonomy_context/vid
Where vid is a number...

I cannot see the block in the block admin page, so I cannot edit/configure it..
If I type in the URL manually: mysite.com/admin/build/block/configure/taxonomy_context/2
The page title shows '' block . where '' should have said 'Context for Vocab-Name' block
It's funny that this page, even though it doesn't show the title, had the correct user visibility restrictions.

Something happened and caused this one block to get corrupted and disappear. Why did my block for the context for this vocabulary go away?

I went to the Modules page - Deactivated Taxonomy Context and activated it again. The missing Block showed up. Issue fixed.
