Token replacement and return link in renewal e-mail broken

When the module sends out renewal e-mails, specific tokens should be replaced by the correct values. This is not working the way expected in the current version.

Secondly, the renewal page isn't working because the ec_recurring_renew_item() function is evaluating the input $token against a non-existing object.

The patch provided should fix these issues.
