I've applied email.module_4.patch so I'm no longer getting access denied, but when I click the link I get "The requested page could not be found." Is there some other configuration I need to do? See http://kingsleyre.com/drupal-6.2/agents for live example.

Figured it out. You have to have the setting set in the cck template and in the form you fill out.
