1) Go to http://drupal.org/project/[someproject].
2) Click the Edit tab.
3) Click either the Issues or Releases subtab.
4) Look at your 'user' block and you'll find 2 empty links. These links are actually the "delete" and "revert" links.

Note: I'm on the documentation team, perhaps it only happens in that case?

Confirmed, and I'm not on the doc team, so it's not related to that. Also note, 'my projects', and 'my accounts' are swapped and 'create content' is moved as well.

I think this was a cache_menu issue, and I haven't seen this anymore, or heard any other reports of this.
