If I want to change the look do I just hack advanced_forum.css in my theme/advforum folder? (in my case it is zen)

If you only need to make minor changes, I recommend just overriding the styles in your style.css. That way you can just drop in a new file when you update advforum without having to worry about your changes. If you need to make extensive changes, though, then, yeah, modify that file.
