This module seems to be quite similar to the actions and triggers ideology. Does the Conditions API hook into the Actions API so that you could use the Triggers module's UI to trigger certain actions when a Condition from hook_condition is met?

It's a good idea but I think it's out of scope for this module. Please check out Rules though - it does just this.
