Existing audio files not showing in list

Sometimes previous files do not appear at all. Sometimes they appear with their node name. Sometimes they appear blank as in this picture. Have tried uploading as audio nodes and as attachments to page nodes. I even manually created the audio_file table in the database as it seemed to be missing every time I uninstalled the modules. Audio-attach is a very frustrating module for me because its exactly what I need, but it doesn't work (on my site at least.). Thanks for all the hard work so far though - really nice implementation of the flash player...
