Installed in expert mode, then just visited the 'admin/settings/filters' page and received those notices. Also, nothing appears in the Name field, but the rest of the fields were filled properly. Should we be using #markup instead of #name? I changed this on filter.admin.inc, and I did not receive any notices and the name field was filled out properly.
