I found a logic bug in AvatarBlocks that prevents the first imagecache preset from being applied. Imagecache returns the list of preset names alphabetically, so this effects the first alphabetic preset name in the list. This means that if you have only one preset created, it does not get applied.
I've fixed the bug and am about to commit the changes... I just wanted an issue number for the CVS log.
