I created a back-end system for a doctor's office and now that it's all done they decided that they are unhappy with the way the forms look. What they do is print out the forms and use them to enter the info into another DB that for security reasons isn't connected to the Internet.

You can see what the forms look like by going to http://www.greenspoonortho.com/gso-forms/ and signing up for an account (DO NOT SUBMIT ANY FORMS, THIS SITE IS LIVE).

I've got the printer friendly link mod installed that just strips all the formatting and returns:

Question: Answer

But what they want it to look like is this:

but with the answers filled in.

Is there any way to do that?

There old system used MS Access as the back end and they wanted to update to a more portable format. I did say that things wouldn't look exactly the same but it would be close.

What say you?

Thanks,
David

PS - Also, if you could let me know in this thread if you signed up for an account what your user name is so I can delete the account after you've looked around that would be great.

If your forms are CCK based you can theme the output so it looks like what you have shown.

honestly? i would fix the forms. I don't see anything on that form that can't be done with cck and contemplate. But i admit, for something like this, i would absolutely use tables for layout. You'd also probably have to theme the print.css carefully as well, but it shouldn't be that difficult.

Also, you won't find many people willing to sign up for an account to see what they need to see to help you out. It's best to include a link to a screenshot you post somewhere (like you did with the pdf).
