Not working.. Field present regardless of parent state.

Issue Summary

I tried to no end to get this to work. No luck.

I have radio button with 3 options..
fax|fax
email|Email Attachment
mail|US Post 1st class.

Fax number field is supposed to be dependent on fax|fax radio selection. It appears no matter what selection is chosen. Furthermore it appears where the weights set it, NOT after

When I just use a single checkbox for a value it works fine.
