Attached patch creates an 'up' navigation link that moves up to the parent item in the tree (if available). I've tweaked the CSS a bit so that it handles it gracefully when there's no siblings available (left or right). I'm no CSS master, as we know, so I borrowed a bit straight from book for it.

I left out 'up' intentionally, as the breadcrumb also has it. Maybe that was the wrong idea, but it always felt kind of weird to me to have an up there.

I had the same request - I found it hard to get back up, e.g. to the TOC

Relented. Modified a bit to fix a couple of minor issues and also allow "up" to go back to the main topic list which is one level above the help.
