Administer > Site Building > Themes > Configure > Global Settings/Garland

Why can't I enable "User pictures in posts" and "User pictures in comments"? The check-box is gray and unclickable.

Furthermore, how can I select a profile image? I uploaded an image using the image module, but I don't see an option to make it a profile image or anything like that.

You'll need to enable Picture Support at Administer >> User Management >> User Settings.
