Invoking a Hook when modr8 action is called.

Issue Summary

Hi there,

I need to be notified when a moderation action is called, and I added a couple of lines of code to make this feature available.

(hope the patch file works!!)

committed the attached patch - note I changed the hook name to reflect better it's purpose/connection.
