I got this after activating URL icon filter in Input formats today..

this went away after refreshing the page.
When i checked Log entries there were too many entries saying could nor=t find fav icon for URL...one was
urlicon

Can't reproduce the MySQL issue.
But fixed the "Page not found" issue.
