Move feed source info into title

I'd like to move the feed's source into the title. This makes more sense IMHO, especially for Planet-like aggregators.

The attached patch removes the "Source: Foo" from the bottom of all aggregator items and changes all titles to read "Foo: Title-of-the-aggregated-item". The "Foo" in the title links to the aggregator source page of the item.
