Let me throw my own humble store in. We have all of 2 themes currently so selection is poor and our first theme doesn't contain any of the neat things the second does so it is priced accordingly. As far as I know our themes are the only Drupal themes available with built in tabs and drop downs etc. Any theme purchased from our store are available for life via the free updates system so if a problem is found in a theme by our users we can address it and provide the changes to all of our users for free.

Yes but this is very expensive.

One or more designers have to work on the graphic design for you site based on your requirements, and then one or more programmers familiar with Drupal have to build the theme in xHTML + CSS + PHP (+ JS). Depending on their experience level and their attention to detail, the job may take anywhere between 15 to 50+ hours.

There is a handful of companies that specialize in Drupal and Drupal theme development. I can't speak for anybody else, but with our company a custom theme budget will be no less than $1,450, which reflects an average of 20-25 hours of work per custom theme:
