I used the following pattern for all page paths: Our_Ministries/College_and_Career/[title-raw]. In place of [title-raw] all I get is "page" instead of the node title.

Sorry for not responding sooner. I'm not sure why or how this could be happening. I've never seen it before and if it were happening more broadly then I imagine we'd get lots of reports about it (moving to critical because of that).

Can you install a brand new site with brand new copies of Pathauto and Token (and no other modules) and see if the problem happens again? If not, compare the new site to the site with the problem to see what might be causing the problem.
